Below is the information on how to register for the upcoming Wolverines Football/Cheer season....
ONLINE REGISTRATION & PAYMENT:
Online payment with a debit/credit card
SUBMIT ADDITIONAL REGISTRATION FORMS:
Download, complete and collect the following forms (available under the Registration Documents tab) and submit to the Player Agent the earlier of first day of practice/equipment handout.
Medical Clearance Form
Copy of Birth Certificate (not needed for returning players)
Headshot of player/cheerleader (an updated headshot is required every year)
A Team Football ONLY - report card or proof that player still attends Middle School
Code of Conduct Form (signed by player/cheerleader and parent)
There will be an additional $50 pymt required to cover volunteer tasks. Checks will be returned when your volunteer work has been completed.
NOTE - ALL paperwork MUST be turned in so your player/cheerleader can receive their equipment and/or uniform!
