8^th Grade Scholarship Opportunity

The St. Matthias Athletic Committee will be awarding up to (4) $500 scholarships to 8^th grade students attending Catholic High School and up to (4) $250 scholarships to 8^th grade students attending public high school, but continuing in the St. Matthias Religious Education program. $2500 total will be given out to graduating 8^th grade students.

The Scholarship is named in honor of Jeff Johnson, a long time committee member, coach, and St. Matthias parishioner for his contributions to the improvement of the St. Matthias athletic program. The winners will be announced at the Spring Athletic Banquet and again at Graduation in June. In addition, the winners will have their names enshrined on a plaque to be displayed in school for years to come. The athletic scholarship committee will review the applications anonymously, discuss, and select the winners based on the criteria below.

Jeff Johnson Athletic Scholarship Requirements:

• Must be attending a Catholic High School or St. Matthias Religious Ed

• Must have participated in Athletics  during 5^th – 8^th grade (At minimum one sport, for one year)
• Completed application emailed to by Monday, April 20, 2020

4. Type your answers into the space provided, save the file, and attach your saved file to an email to
