To Submit approval online (preferred): 

1. Sign on as you did when you registered with your user name and password. 

​2. Click on your daughters registration link and 'edit' to view waiver/release form. 


To Submit in person:  Please Download, Print, Review & Sign. Bring copy to the first practice.

Refunds

• Full refunds available 2 weeks or more prior to program start - minus any out of pocket costs incurred.
• Partial refund available less than 2 weeks prior to the program start - in addition to any out of pocket costs
• No Refunds after program starts - unless for valid medical reasons. Some exceptions made on a case by case basis.
