2013 Season - Financial Aid Application Process

 
To apply for financial assistance you must complete and submit an application. For a copy of the 2013 Financal Aid Application Form please send an email request to the SMLC President at  A form will be sent to you by email along with an access code that will allow you to complete the online registration process and avoid being placed on the player registration waitlist.  

SMLC has limited funds available for financial assistance.  SMLC reserves the right to award no financial assistance and to terminate this program at any time.  All submitted information will be kept confidential and will be used solely for this application request.
