Templates and Forms

Templates and Forms are available for the following:

Rosters - for labels and for laminated cards  This is a Word Document

Use 2 x 4 mailing label (like Avery 5163 or Avery 5963).  This Word template is formated to be printed correctly.  These may also be available from the IYHA Registrar at the beginning of the season using the USA Hockey Cybersport program.  If you need assistance, contact the Registrar

Rosters - for labels and for laminated cards  This is a Microsoft Excel file

Labels are needed for the scorebooks, otherwise you have to manually write the player's name and number through 4 carbon copies at each game

You can't use 2 x 4 mailing label because the Excel spreadsheet is landscape, not portrait.  Use a full sheet label (like Avery 8165) then cut them into the individual labels.  These may be available from the IYHA Registrar at the beginning of the season using the USA Hockey Cybersport program.  If you need assistance, contact the Registrar

Coach labels  This is a Microsoft Word document

Coach labels are an additional set of labels that contain the Coach's information used at the bottom of the scoresheet.  You will need 4 at each game (for each carbon copy)  This is for the area on the scoresheet that requires their name and CEP numbers along with the Team Manager's name and phone number (and a signature line for the head coach).  Having these labels will save you a tremendous amount of running around prior to each game

Add the names and CEP numbers after the colons.  Leave the line under the head coach for a signature line.
