GLL Forms

For the 2023 season, the league is collecting forms and waivers electronically. Your individual association has the links to the forms that they can provide. It takes less than 5 minutes to fill out. We apologize for some redundant data that you may have to enter on the forms and in the Associations registration system. There may come a time when we will have a common registration system and this will no longer be an issue.

All of the forms that are required can be found on your Associations website. If you have any questions about certification for 2023, please contact your GLL Certification Manager.
