Accident Claim Form Instructions *- To expedite league personnel's reporting of injuries, we have prepared guidelines to use as a checklist in completing reports. It will save time -- and speed your payment of claims.
Accident Notification Form *- This form must be completed by parents (if claimant is under 19 years of age) and a league official and forwarded to Little League Headquarters within 20 days after the accident.
Little League Constitution 2016 revision *- MYBBSB revised its constitution with Little League International. The constitution is the document that governs the charter of Moose Little League with LLI. It pertains specifically to Little League requirements.
Medical Release Form *- To be carried by any Regular Season or Tournament
Team Manager together with team roster or eligibility affidavit.

2010 Code of Conduct Guidelines- Middletown Youth Baseball & Softball's Code of Conduct policies for the 2010 season. The form must be completed, signed and dated by ALL adults involved in the league -- parents, coaches/managers, volunteers and board members.

2010 Volunteer Letter- Do you have 2 hours to spare over a 3 month baseball or softball season? By contributing as few as 2 hours over an entire season by volunteering, you can make a difference and help keep Middletown Little League operational.
Middletown's Longest Game *- On June 19 and 20, your child can make baseball history AND learn about helping others. This is the date of our very first 24-hour baseball game, "Extra Innings". It is also the cornerstone of our 2010 fundraising effort. Proceeds will benefit the Mia Hamm Foundation and Middletown Youth Baseball & Softball. Process will go toward the addition of lights to our playing fields.
