Coach/Volunteer/Parishioner Accident Report Form 

This form is to be used to report accidents to Catholic Mutual that involve individuals not included in the Archdiocese of NY Student Accident Insurance Program,  such as employees, teachers, visitors, parishioners, and volunteers.  Use the Student Accident Report Form for individuals included in the Student Accident Insurance Pogram.   

For CYO, the Coach or Parish Coordinator should fill out this form and follow parish policy to have the form faxed to Catholic Mutual Group. This is not a form for the injured party to complete.
